* If the entry form is not completed and signed your entry will be disqualified

* One entry per contestant

* Only One (1) Winner of $100 will be Awarded

* Winner will be announced April 15, 2021 at 8PM

* Award will be mailed to the address provided on the entry form by April 18, 2021
